Change Of Address Within Same Juridiction In Ration Card :

You can apply in the form no.D-1 for change of address with same jurisdiction. Clicking Here on form no. D-1 , you can see a copy of form no. D-1 .

You may download the same and present the duly filled in hard copy at your Circle Office along with the Ration Card. The amendment, after the required verification, will be done within 7 days.

** Form attested by the officer of the department if a person on Government employee.
** In case of non-government employee form will be attested from any gazetted officer or MC.,Sarpanch, Panch, Lambardar of the concern district.
** Original Ration Card.

Procedure :
The applicant should put up his/her application to circle FSO / e-Disha center where the applicant permanently resides, on any working day, and obtaining and Acknowledgement Receipt for the same. The applicant is also given a delivery date for his/her certificate, which is printed on the Receipt. Applicant will receive the Ration Card from the Receipt Counter OR though Courier or Speed Post

Fee Details :
** Total Fee = 10 /-
** If delivery through Courier or Speed Post then extra Fee 25/-
** Expected Delivery Period – 3 working days

Work Flow Process For Change Of Address Within Same Juridiction In Ration Card
** Applicant submits the application form containing all the necessary documents at block / Tehsil level e- Disha Centre
** Necessary Document 1. Original Ration Card. 2. Application Form ( Attested from Sarpanch / Panch / MC / Gadgeted officer )
** then Summit Preliminary Scrutiny of Documents
** Facilitation desk will help to remove the discrepancy
** If file is Not Ok Applicant will be informed about the discrepancy
** If file is ok A fixed Service Fee will be collected from Applicant at E-Disha’s Counter
** A computer generated receipt with a unique ID will be given to the applicant with a target date
** next For Verification Of Address… Documents conveyed to AFSO / Inspector by DLEDC / TLEDC on the next day
** IF ADDRESS NOT VERIFIED Application Rejected and information sent to applicant through SMS / Speed post
** IF VERIFIED Ration card will be prepared by AFSO / Inspector and will be submitted at the Receipt Counter of Tehsil / Block Level
** then Applicantwill receive the Ration Card from the Receipt Counter OR though Courier or Speed Post
