Instruction for filing e-form for change of name in ration card
** E-Form for Change of name in Ration Card can be filled Online as well as Offline.
** The Change of name in Ration Card is available in English and Manipuri (Bengali script) languages, you have to change the portal language to get the same.
** For filling up the Ration Card Online / Offline, you need to be a registered user with State Portal of Manipur.
** You will be required to click on “Apply Online” button for filing the form Online or “Download Offline Form” button for filling the form Offline.
** For filling the details either Online or Offline, you can change the name of head of the family and give the reason for the same.
** Citizen has to attach Income certificate of the head of family, Recommendation from the Panchayati Raj institution or Municipal council and Proof of change in the name. All the other fields values in the form will only be read-only and cannot be edit.
** All fields marked as * on E-Form are mandatory fields.
** As Ration Card will be issued from concerned Authority in your District jurisdiction, you are required to choose the correct District in the field “Please select the district where you want your form to be processed” failing which there may be a delay in your application processing or your Application may be rejected by Authorities.
** On submitting the form Online as well as Offline, you will be required to choose the payment medium out of given Cash / Cheque / Receipt modes which you would be required to submit by physically visiting the Department or your local CSC Dealer.
** Any wrong / misleading data entered by you in E-Form is liable to get rejected by Department Authorities from your concerned District Jurisdiction.
** By filing up this form you assure Department that whatsoever information is filled in e-form is valid information for getting Ration Card.
** After filling up the E-Form, you will be getting Transaction ID, which you can take for future references or tracking your application status. For any further communication with Department Authorities for your E-form, please refer to the corresponding Transaction ID only.
** If Login ID and Password entered in by you is correct, your form will be submitted to Department.
** Besides submitting the form, a print-out of the form can also be taken, if required, on click of print button on the e-from.
