Eligibility:
** An Oral and Maxillofacial surgeon may become member of the Association provided he/she has applied for becoming a member in the prescribed form with the following qualification:
** Dental practitioners registered under Part A of Indian Dentists Act 1948, and possessing a post graduate qualification in the specialty of Oral and Maxillofacial surgery from the University/Institution recognized by Dental Council of India.
** An application should not be engaged in any activity detrimental to interest of the association . He should not have been convicted by any competent authority for any criminal offence. He should not be of unsound mind or insolvent.

Now application for ISA Life membership is online. You have to log in to official website and click BECOME A MEMBER –> Form .
You should have soft copy of the following in hand
** Graduadtion (BDS) for All
** Post Graduadtion (MDS) for All Except Student Life Member
** Bonafide Certificate for only Student Life Member
** Others Certificate if any (for all)
** Good quality Passport size color photo (for all)
