What is Registration of Birth and Death Act?
The registration of births and deaths is carried out under the provisions of the Registration of Birth and Death (RBD) Act, 1969 and the Registrar General, India is entrusted with the responsibilities of co-ordinating and unifying the activities under CRS in the Country.

Eligibility
Birth Registration:
Parents(Father/Mother) or any relative of the child can apply for birth registration, Also the event can reported by local government bodies(i.e. Anganwadi Workers,ANM’s, ASHA’s).
Death Registration:
The death event can be reported by the Head of the Family or relative of the deceased, Also the event can reported by local government bodies(i.e. Anganwadi Workers,ANM’s, ASHA’s).
How to Register Birth & Death in Odisha?
Registration of Birth/Death Odisha:
For birth and death registration citizen are adviced to take hospital acknowledgement slip before hospital discharge.
Births in a Hospital or Nursing Home:
Any person authorised by the medical institution is eligible to register the birth.
Birth in a house:
The head of the household is eligible to register the birth with the concerned authority.
Birth in a public place:
The head of the corresponding village or the officer in charge of the local police station in the area is eligible to register the birth.
Note:
** A death can be reported and registered by the head /guardian of the family to the concerned authority in case the death occurs in the house.
** The death can be registered by the medical in charge if it occurs in hospital.
** For applying death certificate , death has to be registered first to the concerned authority of the locality

** For birth and death registration citizen are advised to take hospital acknowledgement slip before hospital discharge.
** If registration is done within 21 days of birth and death the first certificate fee will be free of cost.
** Citizens are requested to give valid mobile number and email ids for various services to get system generated information on status.
** Digital Signature and QR coded Certificate being issued in the Application.
** Citizen being intimated about the status of the application via SMS.
** Citizen are requested to take Digital Certificates from the Application

Following documents are required for Odisha Birth & Death Registration
Birth Registration:
If the baby has born in hospital, then the Hospital Discharge Certificate and Government ID proof (i.e. Aadhar or Pan card or voter ID or Driver’s Licence etc.) of the person registering are needed. If the baby has born in home(not in hospital) then written letter from local authority(i.e. Sarpanch) along with Government ID proof (i.e. Aadhar or Pan card or voter ID or Driver’s Licence etc.) of the person registering are needed.
Death Registration:
If the person died in hospital, then the Hospital Discharge Certificate and Government ID proof (i.e. Aadhar or Pan card or voter ID or Driver’s Licence etc.) of the person registering are needed.
