** Click on “Online Application for Empanelment of Hospitals” as shown in Screen Shot-I

Empanelment Application. :
** On clicking the Online Application for Empanelment of Hospitals, separate window will be displayed as shown in Screen Shot-II where you can find the Eligibility Criteria for empanelment of hospitals.

** If you scroll down the Eligibility Criteria for Empanelment of Hospitals the window will be displayed as shown in Screen Shot-III where you can see the options to apply the Fresh Application Form(Normal/Dental/Psychiatry) or already signed up Application Form.
** Click on Normal/Dental/Psychiatry Application- if you are applying for Empanelment for the first time.
** Click on Signed up Application(Old/New) -if you have already applied and.. Reference no & PIN No for the further processing of the Application.
Note :
Once the hospital applies for Empanelment by clicking on Fresh Application and for the next time when the same hospital try to click on Fresh Application and Fill the Hospital information then the system will not allow and pop-up will be displayed as “Hospital is already signed up. Please use same hospital reference number to update the details. Contact EHS Empanelment department for further clarifications”
Hospital Basic Information
** Update the Hospital Basic Information and Hospital Mandatory Approval Details
** Enter hospital Name, Plot No, Location, Street, Pin code, Telephone number, Fax No, Mobile number of MD / CEO and E- Mail ID.
** Select the Existing/New Hospital, Government/Corporate, City name and District from the dropdown.
** Enter the total Bed Strength of the hospital.
** Enter the Hospital premises details as shown in screen Shot IV.
** Update the Hospital Infrastructure details and Financial details.
** Please enter the site owner ship details, site area, plinth pare, no of floors, lift provision, ramp provision, classification of beds and rooms.
General Infrastructure :
** Basic Units- “1a Genaral” Infrastructure details to be filled as per the screen shot XVII.
** Basic Units- “2a Genaral Wards” infrastructure details to be filled as per the Screen shot XVIII and also attach related documents.
** Basic Units- “3a Operation Theatre” Infrastructure details to be filled as per the Screen shot XIX and also attach related documents.
** Basic Units- “4Aa diagnostic center – radiology(Basic)” infrastructure details to be filled as per the Screen shot XX
Upload the filled Application :
** form by click on Browse button and upload as shown in Screen Shot CVI. The hospital information Uploaded successfully.
** After upload the Hospital Information, Click on the “Show Online Application form” to upload the attachments like Declaration form, MOU with insurer, Trust, e-copy etc.
** After click on “show online Application form” please go to from Screen Shot XCV to Screen Shot CI.
