How to Apply :
Online form Instructions :
** On the State Portal of Punjab, click on Citizen Login link.

** If you are a new user, click on “New User” link, else login with your Username and Password.
** After logging in for the first time, user will be asked to update their profile. Please update it.
** Click on “Fresh Application” link in the left menu.
** On the next screen, all Department wise services are listed. Choose the service you want to apply for and click the “Apply” link next to the service.
** On the Fresh Application screen, go to Alternative 2(Online Form application) section and click on “Click here to fill the application form online” link.
** Fill the form and click “Submit”.
** Click “View Saved Application” link, select the application and click “Upload Supporting Documents” button.
** Upload the supporting documents.

Offline form Instructions :
** On the State Portal of Punjab, click on Citizen Login link.
** If you are a new user, click on “New User” link, else login with your** ** Username and Password.
** After logging in for the first time, user will be asked to update their profile.** Please update it.
** Click on “Fresh Application” link in the left menu.
** On the next screen, all Department wise services are listed. Choose the service you want to apply for and click the “Apply” link next to the service.
** On the Fresh Application screen, go to Alternative 1(Offline Form application) section and click on Form Name to download the e-Form.
** Fill the e-Form in offline mode (Internet connection is not required to fill e-Form).
** Login to the application and on the Fresh Application screen, go to Alternative 1(Offline Form application) section and click on “Click here to upload the filled form” link.
** Browse to the filled e-Form and click on “Add” button.

Documents Needed :
1. Residence proof :
** Voter Card OR
** Driving Licence OR
** Bank Passbook OR
** PAN Card OR
** PassportOR
** Telephone, Electricity, Water bill, indicating the address of the applicant
** OR Aadhar Card
** Certificate of residence issued by a panchayat, Municipal Cooperation Cantoment board and gazetted officer or the Concerned Patwari or Head Master of Govt. School With disabilities, mentally ill, etc., a certificate of residence
2. Scanned Passport size photograph

About Punjab :
The first known documentation of the word ‘Punjab’ is in the writings of Ibn Batuta, who visited the region in the fourteenth century. The term came into wider use in the second half of the sixteenth century, and was used in the book Tarikh-e-Sher Shah Suri (1580), which describes the construction of a fort by ‘Sher Khan of Punjab’.
