How to Apply :
Online form Instructions :
** On the State Portal of Punjab, click on Citizen Login link.
** If you are a new user, click on “New User” link, else login with your Username and Password.
** After logging in for the first time, user will be asked to update their profile. Please update it.
** Click on “Fresh Application” link in the left menu.
** On the next screen, all Department wise services are listed. Choose the service you want to apply for and click the “Apply” link next to the service.
** On the Fresh Application screen, go to Alternative 2(Online Form application) section and click on “Click here to fill the application form online” link.
** Fill the form and click “Submit”.
** Click “View Saved Application” link, select the application and click “Upload Supporting Documents” button.
** Upload the supporting documents.
Offline form Instructions :
** On the State Portal of Punjab, click on Citizen Login link.
** If you are a new user, click on “New User” link, else login with your** ** Username and Password.
** After logging in for the first time, user will be asked to update their profile.** Please update it.
** Click on “Fresh Application” link in the left menu.
** On the next screen, all Department wise services are listed. Choose the service you want to apply for and click the “Apply” link next to the service.
** On the Fresh Application screen, go to Alternative 1(Offline Form application) section and click on Form Name to download the e-Form.
** Fill the e-Form in offline mode (Internet connection is not required to fill e-Form).
** Login to the application and on the Fresh Application screen, go to Alternative 1(Offline Form application) section and click on “Click here to upload the filled form” link.
** Browse to the filled e-Form and click on “Add” button.
Name of Documents Need For Death Correction :
** Scanned Copy of Original Death Certificate (self attested)
** Scanned Copy of School certificate of deceased.(self attested)
** Self Declaration (Describe the reason of correction) under state rule
** Witnesses of two credible persons like Sarpanch/ Panch / Municipal Councilor (MC) / Member of the Legislative Assembly (MLA) / Member of Parliament ( MP) or Gazetted Officer under state rule 11. (self attested
** Voter card or Adhaar Card for proof of residence (self attested)
** Scanned Copy of post mortem report and FIR in accidental
Name of Documents Need For Birth Correction :
** Copy of Birth Certificate (self attested)
** Copy of School certificate of child/Father/Mother in case of correction in their name (self attested)
** Self Declaration (Describe the reason of correction) under state rule 11
* Witnesses of two credible persons like Sarpanch/ Panch / Municipal Councilor (MC) / Member of the Legislative Assembly (MLA) / Member of Parliament ( MP) or Gazetted Officer under state rule 11. (self attested)
** Copy of Report of hospital / Nursing home in case of institutional delivery(self attested)
** Voter Card for proof of residence and relationship / Copy of Bank Passbook / Copy of Passport/ Copy of Driving License / Aadhar Card (self attested
