Apply for Driving License :

For Learner License one has to apply before the Licensing Authority along with the following documents :
** Application in Form No.2.
** Medical Certificate in Form No. 1-A.

** Three copies of Passport photographs.
** Fees specified rule-81.
** Proof of age & address.

For the permanent Driving License one has to apply before the Licensing Authority along with the following documents :
** Application in Form No. 4.
** Learner License.
** Three copies of passport photographs.
** Driving Certificate in form-5 issued by approved Training School (Compulsory for Transport Vehicle License)
** Fees as specified in Rule-81.

For the renewal of Driving License one has to apply to the Licensing Authority along with the following documents :
** Application in Form.9.
** Permanent Driving License.
** Medical Certificate in Form 1-A.
** Three copies of passport photographs.
** Fees as specified in rule-81.

PRESCRIBED FEES :
The prescribed fees for various Licenses issued by the department are listed below. (revised vide Ministry of Road Transport and Highways Notification dated May, 31, 2002)

HEAD ITEM FEES :
** Learner License 30.00
** Permanent Driving License 40.00
** Permanent Driving License( in laminated card ) 200.00
** Renewal of DL 50.00
** Driving test 50.00
** Conductor License fees Half of DL fees
** Endorsement of new class of vehicle in laminated card 200.00
** Renewal of DL after expiry of grace period 100+50 (per year)
** Registration / Renewal of Driver Training School 2500.00
** For second copy of Registration / Renewal of Driver Training School 2500.00
** For appeal against orders of the Licensing Authority 100.00

NOTE :
1. These fee rates are subject to change. The details of fee rates should be referred to in the latest amended Central Motor Vehicles Rules, 1989 and UP Motor Vehicle Rules, 1998

2. Where a Smartcard Type driving licence is issued in Form 7, an additional fee of two hundred rupees shall be levied.

3. The fees specified at serial numbers 1, 2 and 3 of the Table above shall be paid collectively at the time of submission of application for issue of learner’s licence or driving licence or for endorsement of another class of vehicle, as the case may be.”
