New Ration Card Procedure

** Application on plain paper.
** Declaration form duly filled and signed by the head of Family.

** Ration Card Cancellation/Surrender/Deletion Certificate from Applicant’s earlier place of residence, issued by the Civil Supply Department of that place.
** Copy of Applicant’s earlier Ration Card which he/she was holding at his/her earlier place of residence.
** The Civil Supply Department, Daman shall get the Ration card Cancellation Certificate produced by the Applicant, verified with the
** i. Attested copy of document like Sale Dead/House Tax Receipt/Electricity Bill/Water connection Bill etc. of the House/Flat to prove that applicant is the owner of the particular house/flat in which he and his family are residing.

** ii. Where House/Flat is taken on rent – N.O.C. with Leave Deed if any from the owner of the House/Flat, supported by attested copy of document like House Tax Receipt/Electricity Bill/Water Connection Bill etc. to prove beyond doubt that the particular House/Flat belongs to the person who signed the N.O.C.
** certificate from the Employer of the applicant, certifying that applicant is an Employee of his Organization/Company/Institution and also certifying applicant’s duration of service and residential address as per their records.
** In case of aged/retired applicant, businessman, etc. where certificate from an Employer is not possible, the Civil Supply Department, Daman shall forward such application to the concerned Talathi and get his/her factum of residence verified.
** Two passport size recent photographs of the applicant/head of family.

Bifurcation Of Ration Card From The Existing One :
** Application on plain paper.
** Declaration Form duly filled and signed by the Head of Family.
** N.O.C. from the holder of the Ration Card.
** Attested copy of document like Sale Dead/House Tax Receipt/Electricity Bill/Water connection Bill etc. of the particular House/Flat to prove that applicant and his family are residing separately from other members of the family in a separate house.
** In cases applicant and his family are staying in the same HOUSE with the holder of the particular Ration Card but their Living Room/Kitchen are separate and has a complete separate establishment, applicant shall submit attested copy of documentary evidence to prove the contents mentioned in his application.
** An Affidavit duly executed by the applicant stating on oath that the particulars mentioned in his/her application/declaration form, for issue of a separate ration card are true/correct.
** In case where applicant is residing in rented house/Flat, N.O.C. from owner of the House/Flat, supported by attested copy of document like House Tax Receipt/Electricity Bill/Water Connection Bill etc. to prove beyond doubt that the particular House/Flat belongs to the person who has signed the N.O.C. [The Civil Supply Department, Daman shall forward such application to the concerned Talathi for verification of (d) & mentioned above]

Addition Of Name In The Existing Ration Card :
For children upto 12 years of age :
** Application on plain paper to be made by the Head of Family/Holder of the Ration Card.
** Attested copy of Birth Certificate of the Child, ISSUED BY Competent Authority whose name is intended to be included in the existing ration card.
** Name of the parents should be existing in that particular Ration Card.
** Where name of a single parent i.e. either father or mother is existing in the Ration card, the application should be supported with an Affidavit duly executed by the applicant, stating on oath as to why the name of spouse is not existing in the Ration Card and Birth Certificate of the Child.
** The name of the parents should tally/match with the names mentioned in the Ration Card and Birth Certificate of the Child.
** In cases where the name of both the parents of the child are not existing in the Ration Card, the applicant shall execute an Affidavit mentioning his/her relation with the child and giving reason why the child is under his/her custody. The civil supply Department Daman shall get such application verified thought the concerned Talathi

For the person(s) above 12 years of age :
** Application on plain paper by the Head of Family/Holder of the Ration Card.
** Ration Card Cancellation Certificate, issued by the Civil Supply Department of earlier place of residence of the person whose name is proposed to be included in the Ration Card along with copy of earlier Ration Card where his/her name was existing. Only certificate issued by the Civil Supply Department will be accepted. Certificate issued by any other authority will not be accepted.
** [The Civil Supply Department, Daman shall get the Ration card cancellation Certificate produced by the Applicant, verified with the issuing authority]
** Attested copy of Birth Certificate/School Leaving Certificate of person whose name is proposed to be included in the Ration Card.
** An Affidavit from the Applicant stating on oath his relation with the person and reason for inclusion of name in his/her Ration Card.
** Documentary evidence to prove that the person is residing in Daman i.e. Certificate from the Employer/School Bonafide Certificate/Attested copy of PAN Card/Bank Pass Book/Election I.D. Card etc. of Daman District.
** In case of married women, attested copy of Marriage Certificate/Affidavit for change of name.
** Application for inclusion of name in Ration Card merely for the reason that his/her name was/were not included earlier in the Ration Card or was included earlier and subsequently dropped/cancelled and desires to be included now, shall not be allowed/considered.

Correction Of Name In Ration Card :

** Application on plain paper
** Attested copy of Birth Certificate/School Leaving Certificate/Mark Sheet/Pan Card/Election I.D. Card/Bank pass Book etc.
** Affidavit for Correction of name.
** Declaration in 2 Local News Papers with Photo Identity.
** Any other correction in the Ration Card i.e. correction of Age/Income etc. shall be carried out on application and supported by documentary evidence.
** Correction of a particular unit will be carried out only once in Ration Card.

Issue Of Permanent Ration Card To Temporary Ration Card Holders :
** Application on plain paper along with Attested copy of Temporary Ration Card.
** Declaration form duly filled and signed by the applicant.
** Certificate from the Employer of Applicant, certifying that applicant is an Employee of his Organization/Company/Institution and also certifying applicant’s duration of service and residential address as per their records.
** Applicant shall submit documentary evidence to prove his duration of stay in Daman District.
** The Civil Supply Department, Daman shall forward such applications to the concerned Talathi and get his/her residential proof and duration of stay in Daman verified.
** Two passport size recent photographs of the applicant/head of family
** Permanent Ration Card will be issued to only those Temporary Ration Card Holders, who are staying in Daman continuously for the last 10 years.

Issue Of Duplicate Ration Card :
** Application on plain paper
** Copy of F.I.R.
** Affidavit stating on oath by the applicant that his/her Ration Card is lost and if found in future, the same will be surrendered to the Civil Supply Department.
** Copy of Lost Ration Card if available.

Renewal Of Old Ration Card Issued In The Year 2000,2005 & 2009 Only Will Be Considered:
** Application on plain paper.
** Declaration Form duly filled and signed by the applicant.
** Affidavit giving full justification and reason for not applying earlier.
** Residence Certificate issued by the Competent Authority.
** Two Passport size recent photographs of the applicant/head of family
