Apply for Ration Card On Receipt Of Surrender Certificate

When you leave or shift residence outside the Circle, you are required to surrender your Ration card for which you can apply in form D-1.Click here on form D-1

Document Required :
** Two copies of group photographs of family.
** Form and photograph on the form duly attested.
** Form attested by the officer of the department if a person on Government employee.
** In case of nonIn case of non-government employee form will be attested from any gazetted officer or MC.,Sarpanch, Panch, Lambardar of the concern district.
** Original Surrender Certificate.

Procedur :
The applicant should put up his/her application to circle FSO / e-Disha center where the applicant permanently resides, on any working day.

Fees :
Total Fee =10
** If delivery through Courier or Speed Post then extra Fee 25/-
** Expected Delivery Period – 7 working days

Note :
The above fee will be collected by the Concerned Official / e-Disha center at the time of receiving application

Work Flow Process For New Ration Card On The Basis Of Surrender Certificate :
** Applicant submits the application form containing all the necessary documents at block / Tehsil level e- Disha Centre
** Necessary Document ** Form D-1 ** 2 Photo PP sizes ** Attested form from Sarpanch / Panch / MC / Gadgeted officer ** Surrender Certificate
** Preliminary Scrutiny of Documents
** Facilitation desk will help to remove the discrepancy
** If File is OK For Verification Purpose… Documents conveyed to AFSO / Inspector by DLEDC / TLEDC on the next day A computer generated receipt with a unique ID will be given to the applicant with a target date A fixed Service Fee will be collected from Applicant at E-Disha’s Counter
** Ration card will be prepared by AFSO / Inspector and will be submitted at the Receipt Counter of Tehsil / Block Level
** Application Rejected and information sent to applicant through SMS / Speed post
** OR Applicant will receive the Ration Card from the Receipt Counter OR though Courier or Speed Post
