Application Form to Conduct CDE

(1) There will be uniform registration fees of Rs. 1200/- and renewal fee of Rs.200/- per annum or Rs.1000/- for 5 years.

The President of M.P State Dental Council were pursuing since longtime of the registration for a period of 5 years at a time instead of annual renewal. Now, M.P. State Dental Council will take up the matter with the State Government for necessary action, so the renewal of registration for 5 yearly can be implemented.

(2) A system of Continuing Dental Education be introduced for dental surgeons registered ant to be registered within the sovereign Republic of India.
(3) The said requirement of mandatory CDE points would be a prerequisite at being registered and licensed to pursue the Profession of Dentistry in India.

(4) All registered Dental Surgeons would need to accomplish 100 points in 5 years (20 points of CDE per year). Out of these20,4CDE points per year would be mandatory and verifiable. It will be essential to achieve a minimum 15 and a maximum of 25 points a year. 10 points for asepsis and infection control and 5 points for dental jurisprudence over a period of 5 years are mandatory.

(5) CDE programme would commence form 1 June, 2009 and all dental surgeons registered to date on the rolls of the State Dental Councils would be subjected to these requirements.

(6) And that for this period the implementation would be on a voluntary self assessment format with each Dental Surg6ons submitting a self assessment proforma at the end of each year to the State Dental Council and being subject to a random audit

(7) The Council also proposed the implementation of a point system for Continuing Dental Education for all registered practitioners under the Dentists act for an initial period of 5 years to be effective w.e.f. 01 June. 2009 and to be reviewed subsequently at the end of this period.

Instructions For Registration :
1) All copies of Mark Sheet, Certificate, Domicile Certificate and Character Certificate, photo ID has to be submitted along with the registration form duly attested by the Gazetted Officer/Dean/Principal.

2) All particulars given above must be filled in by the applicant himself.
3) All particulars should be in neat legible hand.

State Dental Council was formed as per Chapter III of the Dentists Act 1948 by the Govt. of M.P. vide Gazette notification No. 2002/3985 XVII Medical-IV dated 27/04/1964. 10 persons were nominated in this Council.M.P. State Dental Council has been duly Constituted (First time) as per the Dentists Act 1948 and M.P. State Dental Council Rules 1970 on 16/05/1998. Tenure of this Council is completed on 15/05/2013 as per Section 27(1) of the Dentists Act 1948.
