Instructions :
** Applicant should furnish one of the approved documents by the State Government for the purpose of address proof.(Passport, Pan card, Ration Card, Voter ID, BSNL Telephone Bill, Gas Bill, Water Bill, Electricity Bill, Driving License, Aadhar Card)
** Enclose two additional passport size photos along with the application.
** Furnish the proof of date of birth.
** The PVC / PCC issued shall not be used for any other purpose other than the purpose specified in the application.

** Application through online Forwarding the same to outward section from outward section to concerned DPO DCP From DPO DCP to SDPO ACP From SDPO ACP to PS. 15.00 Superintendent of Police
** After verification in PS seding the report to SDPO ACP to DPO in the Cities DCP to COP for final disposal 75.00 Police Inspector/Circle Police Inspector
