Application For Learners License :

** Scan copy of application form

For NonTransport (MCWG/MCWOG/LMVNT/3WNT) :
** Age Proof (X or XII Admit Card/ Minimum VIII Pass Certificate/ Passport/ PAN Card)

Address Proof :
** In case residing in the same district – LIC Policy/ PRC/ Passport/ Certificate from Ward member with Electricity bill of guardian/ Voter ID with Electricity bill of Guardian
** In case residing in different district LIC Policy/ PRC/ Passport/ Certificate from Ward member with Electricity bill of guardian/ Voter ID with Electricity bill of Guardian and additional Temporary address Electricity bill with NOC from owner.

** One passport size photograph
** Medical Certificate in the format available (It needs to be submitted at the time of original document verification)(Mandatory)
** Written consent of Guardian/ Parent (In case of minor if applied for MCWOG)
** Driving certificate from Driving School/ Institute (if any)

For Transport (LMVCAB/ LMVGV/ TRANS/ 3WCAB):
** Copy of one year old driving license of LMVNon Transport
** Age Proof (X or XII Admit Card/ Minimum VIII Pass Certificate/ Passport/ PAN Card)

Address Proof :-
** In case residing in the same district – LIC Policy/ PRC/ Passport/ Certificate from Ward member with Electricity bill of guardian/ Voter ID with Electricity bill of Guardian
** In case residing in different district LIC Policy/ PRC/ Passport/ Certificate from Ward member with Electricity bill of guardian/ Voter ID with Electricity bill of Guardian and additional Temporary address Electricity bill with NOC from owner

Assam e-District :
The Assam e-District Project is a Mission Mode Project under the National e-Governance Plan (NeGP). This project is being implemented by the Information Technology Department, Government of Assam.
