The registration application form should be accompanied with Original and Xerox attested copies of Matriculation certificate ( DOB),Final Year Mark Sheet,Internship Completion Certificate , BDS Degree, Himachali Bonified Certificate ,Proof of address and, Four Passport Size Colour Photographs.

Affidavit if provisional Degree issued . Registration fee of Rs.2500/- (Rupees Two Thousand And Five Hundred Only) by Cash .Registration application forms are available in the office of Council and also on Council’s website hpsdc.org.in at download section.

NOTE : Person has to come personally to the office for Fresh Registration.

Registration fee of Rs.1000/- (Rupees One Thousand Only) by Cash .Registration application forms are available in the office of Council and also on Council’s website hpsdc.org.in at download section.

How To Renew HPSDC Registration?

The names of Dentist, who fail to renew their registration between 1st January to 31st March (offline) and 1st January to 28th March (online) every year shall stand removed . However the names of such Dental Practitioners can be re-entered in the register, if he/she pays prescribe renewal fees, as provided in the Dentists Act.

Renewal of Registration available online and offline. in online renewal users login with his given user ID and Password and then pay prescribe fee by using Internet Banking/ Credit Card/Debit Card. Online payment Gateway charges are added in fees. Offline renewal done at office at office hours and office working days.
