For Obtaining Duplicate Certificate :

Procedure for Duplicate certificates of the course/examination :
** Candidates applying for the duplicate certificates of the course/examination are required to submit an affidavit cum indemnity on the Non-Judicial stamp paper of Rs. 300/- along with a fee of Rs. 200/- by way of demand draft payable infavour of “National Institute of Securities Markets” along with an Application form in hard copy and a recent passport size photograph.

** The Application Form and the text of Affidavit cum Indemnity should be in the format marked as “Format for Request for Duplicate Certificate/ Name change “and as provided on the website.

** The Affidavit cum Indemnity should be on the Non-Judicial stamp paper of Rs. 300/- duly notarized by Notary or executed before Magistrate. All clauses unless specifically mentioned are mandatory. Portions of text marked within [] are to be included only in case of request for duplicate certificate along with request for change in name.

** The duplicate/revised certificate will be dispatched by NISM at the address of the applicant as mentioned in the Application Form.
** For request for change in name due to marriage a letter format provided as “Format for Request for Name Change” must be submitted in hard copy to NISM along with the original certificate issued under maiden name, a copy of the marriage certificate and a fee of Rs. 200/- by way of demand draft payable in favour of “National Institute of Securities Markets”.

** In case, the applicant has misplaced the original certificate in maiden name she may follow the process for applying for a duplicate certificate along with name change.
