Instructions for online submission of Form 68 :
1. Login using the User Id and Password provided to the Dealer for using other e services i.e. user id and password used in eservices such as e-Filing of Return.

2. Upon successful login the user will be redirected to Main Menu page
3. In Main Menu page click the link “Online Form 68”
4. After confirmation of Dealer Information,user is redirected to Form 68

5. Fill up the Form.Upload the scanned copy of the applicable documents in .pdf,.gif,.jpg,.jpeg format.
6. Click Submit.

7. Upon successful submission an Acknowledgement is generated with Acknowledgement Number.
8. Take a print out of the Acknowledgement Slip and Form 68 (Click the button in Acknowledgement Page to get print out of submitted Form 68).

9. To get the Reprint of submitted form 68 click link “Reprint”.
10. To enter the transaction details of Conditional show cause/stay petition Disposal click link “Challan Details of Show cause/Stay Petition”.

Form 4VA For CST Appeal :
** Login using User id and Password as used during any e-service login such as e-filing of VAT return.
** Fill up the Online Form 4VA.
** Click ‘Submit’ button to submit the data entered.
** Click ‘LOGOUT’ to exit.

** Acknowledgment giving Form 4VA with entered value will be displayed for taking print.
** Signed copy of the Acknowledgement to be submitted at Charge office

FAQs :
1. How To Get User-Id And Password After Getting New Registration?
Dealer himself has to create user-id and password through the link given in the website as Generate of User-id providing registration no, pan no and the mail-id recorded in database.

2. How To Know The User-Id Only?
In case the dealer forgets or loses his user-id, the dealer himself can know his user-id through the link given in the website as Generate of User-id providing registration no, pan no and the mail-id recorded in database.

3. How To Change Password ?
There are two options to change the password :
i) Through Change password link only available after login : dealer can change his password after successful login through the link provided at top left side of the screen appeared.

ii) Using forget password link available in the normal login screen : dealer can change his password through this link himself providing hint question and the answer. On successful matching system will sent a temporary password along with assigned user-id to the mail-id recorded in database.

4. How To Get User-Id And Password- In Case It Is Forgotten Or Lost?
Dealers are requested to approach his Charge/Group Officer and apply for “resetting” his password along with an undertaking that he would be responsible for any action arising out of the use of such reset password.

When the Charge Group Officer is satisfied about the bona fide of the application, the Charge/Group Officer would write an email to the Helpdesk at <cthelpdeskwb [AT] nic.in> for resetting the password. Accordingly dealer will get the Temporary Password to the mail-id mentioned.
