** Assistant Director (CS&CA), Mayabunder for residents of Mayabunder Tehsil.
** Circle Office, Diglipur (Tehsildar, Diglipur) for residents of Diglipur Tehsil.
** Circle Office, Little Andaman (Tehsildar, Little Andaman) for residents of Little Andaman Tehsil.

** Copy of Newspaper Advertisement about lost Ration Card

** Surrender Certificate(s) in original
** Parental Family Identity Cards in original
** Self Declaration
** Certificate From Pradhan / Ward Councilor
** No Objection Certificate by Head of Family of Parental Family Identity Card (Ration Card) for inclusion of additional members (Attested Copies)
** Local Certificate / Islanders Card
** Proof of Date of Birth
** Birth Certificate / X Pass Certificate / declared / other)
** Proof of Residence
** Photo Identity Islander Card / Election / PAN / Driving license card / Passport)
