Apply for Surrender Certificate :

** Surrender Certificate is issued to applicants willing to surrender their passport at Regional Passport Offices mainly for renunciation of Indian citizenship and acquiring foreign nationality.

** Note Individuals applying for Surrender Certificate with any reason other than ‘Acquiring Foreign Nationality/Renunciation of Indian Citizenship’ need to visit the Passport Office directly i.e. without filling the application form physically or online. Examples of such applicants may be the Diplomatic/Official passport holders willing to surrender their ordinary/diplomatic/official passport.
** The downloaded form should be printed back to back on standard A4 size papers and fill the application form in ink.
** Bring printed application form, self-attested photocopies and original documents (e.g. Passport and Proof of Foreign Nationality etc.) while visiting the Passport Office (PO) corresponding to your present residential address for submitting your application.

e-Form Submission
** Step 1 Download the e-Form through the “Download e-Form” link available on the Home Page.
** Step 2 Fill the downloaded e-Form and click the Validate and Save button in it.This will generate an XML file, which will be required later for uploading in the system.
** Step 3 Register through the Passport Seva Online Portal. (Click on “Register” link under the Apply section on the Home Page).
** Step 4 Login to the Passport Seva Online Portal with the registered Login Id.
** Step 5 Upload the XML file (generated in Step 2) through the “Upload e-Form” link.Do not upload the PDF form at this stage as only XML file is accepted by the system.
** Step 6 Take the printout of filled application form through the “View/Print Submitted Form” link available on the View Saved/Submitted Applications page.
** Step 7 Bring printed application form, self-attested photocopies and original documents (e.g. Passport and Proof of Foreign Nationality etc.) while visiting the Passport Office (PO) corresponding to your present residential address for submitting your application.

Online Form Submission :
** Step 1 Register through the Passport Seva Online Portal. (Click on “Register” link under the Apply section on the Home Page).
** Step 2 Login to the Passport Seva Online Portal with the Login Id created in Step 1.
** Step 3 Click “Apply for Surrender Certificate” link.
** Step 4 Fill in the required details in the form and submit.
** Step 5 Take the printout of filled application form through the “View/Print Submitted Form” link available on the View Saved/Submitted Applications page.
** Step 6 Bring printed application form, self-attested photocopies and original documents (e.g. Passport and Proof of Foreign Nationality etc.) while visiting the Passport Office (PO) corresponding to your present residential address for submitting your application.
