Necessary particulars relating to my appointment in the establishment are given below.

** Residential Address in full with telephone number
Related / Similar Service :
KSEB Application for New Electricity Connection Kerala
** Name of the Board Employee & Designation (Capital Letters)
** Relationship of Nominee with Board employee
** Date of appointment in KSEB Other Services if any
** Date of termination of Service
** Amount of DCRG received as per KSR
** Whether case filed for balance amount of gratuity
** If Yes, GC No : Principal amount received with date Interest
Declaration :
I hereby authorize the Board to recover the excess amount if any received by me over and above the gratuity applicable as per the prevailing rules and regulations of the Board directly from the arrears of pension, Arrear of DR and pension.
Signature/Thumb Impression of the Nominee :
** Amount as per DCRG paid
** Amount as per Gratuity Act paid
** The amount if any recovered/withheld towards the excess amount above to the applicable gratuity.
