What is AB-MMSBY Yojana?

Ayushman Bharat – Mukh Mantri Sehat Bima Yojana (AB-MMSBY), was launched on 20 August 2019 this is a flagship State Health Insurance Scheme for the beneficiaires of State of Punjab. AB-MMSBY will provide financial protection to 65% population of state of Punjab. It is entitlement based cashless Health Insurance cover of Rs. 5 Lakh per family per year. Under this scheme,cashless and paperless treatment is available at Government and empanelled Private hospitals.

The cost of premium for 14.65 lakh SECC beneficiary families shall be borne by Central Govt. and State Govt. in 60:40 ratio. The premium cost of remaining 24.53 lakh beneficiary families shall be completely borne by the State (State Treasury & Departments).

How To Search AB-MMSBY Beneficiary ?

Beneficiary Search for entitlment under AB-Mukh Mantri Sehat Bima Yojna (AB-MMSBY)

Step-1 : Go to the link https://sha.punjab.gov.in/shapb/publicPages/benSearchView.php
Step-2 : Enter the Ration card number /PAN number/ Registration ID/Farmer or Jouranlist id, as per search criteria
Step-3 : Enter the Captcha
Step-4 : Click On Search button

Beneficiary Search Criteria :
** Ration Card Holder If your ration card no is like ‘FAZ1234567’ then replace FAZ with 03000 and search with ‘030001234567’, if it is like MNS9809898 then replace MNS with 03000 and search with 030009809898.
** If your ration card is old, contact your local depot holder for new smart ration card number.
** Farmer (j_form holder) Search ID will be First Three Character of District Name + last 6 digit of aadhaar + first four character of your name. For example: if your name is Mohan from District Hoshiarpur with last 6 digit of aadhaar as 123456, then search id will be like: HOS123456MOHA
** Construction Worker Search ID will be the registration id issued by BOCWB, Punjab.
** Small Trader Search ID will be the PAN card number
** Yellow card and Accredited Journalist Search ID will be First Three Character of District Name + last 6 digit of aadhaar with hypen after 3 digit + first four character of your name.
** For example: if your name is Harwinder from District Jalandhar with last 6 digit of aadhaar as 123456 with hypen (-) in between, then search ID would be: JAL123-456HARW

Documents Required For AB-MMSBY E-Card Generation

** Aadhar Card (Mandatory for online KYC. Aadhar not to be uploaded in BIS anywhere)
** Family Identification Document
** Ration Card (Where family members name available)
** If ration card not available, then Family Declaration form signed and stamped by Sarpanch/ Namberdaar/ Municipal Councillor.
** Construction worker registration card (if family member details available in the card)
