GHMC Issue Of Disability Certificate

About Disability :
** The persons with disability from U.T. Chandigarh can apply for disability certificate by either having an application form from the office of department of Ophthalmology, Block-C, Level III on any working day or else it can be downloaded from the website of GMCH,

The duly filled application form may be submitted in the department of Ophthalmology on any working day along with the OPD card.
** The persons with ENT disability should get themselves examined by Dr. Surinder Kumar Singhal on Tuesday & Friday, Block B, Level III, ENT OPD from 9.00 to 1.00 pm.
** The persons with eye disability should get themselves examined by Dr. R.K. Bansal on any Tuesday and Friday, Block B, Level IV, EYE OPD from 9.00 am to 1.00 pm.

** The persons with neuro disability should get themselves examined by Dr. Arvind Malhotra on Tuesday & Friday, Block B, Level IV, NEURO OPD from 9.00 to 1.00 pm.
** The persons with ortho disability should get themselves examined by Dr. Ravi Kumar Preenja on Wednesday, Block B, Level III, ORTHO OPD from 9.00 to 1.00 pm.
** The examination report on the outdoor ticket, application form and patient’s four passport size photographs are to be filed with the office of the department of Ophthalmology, ‘D’ Block, Level III. Thereafter within 7 days, the disability certificate will be ready and the candidate can receive it from the EYE office after depositing Rs.30/- for general patients and Rs.690/- for MACT cases.

** Dr.Arvind Malhotra : Asstt.Prof., Dept. of Neurology Member
** Dr.Ravi Kumar Preeja : Asstt.Prof., Dept. of Orthopaedics : Member

NOTE :
The residence proof and identification proof should be brought at the time of submitting the application form.

Instructions for the Applicant :
** Disability Board meeting will be held on first Friday of every month.
** Please bring three passport size recent photograph alongwith all the old hospital record (if any).
** Please attach the photocopy of the GMCH-32, OPD card.

Fees :
For Court case(MACT purpose)/Claim case Rs. 690/- and for other purposes i.e., travelling, pension & Income Tax benefits etc. Rs. 30/-.
