Issuance of Job Card under MNREGS (National Rural Employment Guarantee Act Scheme) :
Online form Instructions :
** On the State Portal of Punjab, click on Citizen Login link.
** If you are a new user, click on “New User” link, else login with your Username and Password.
** After logging in for the first time, user will be asked to update their profile. Please update it.
** Click on “Fresh Application” link in the left menu.
** On the next screen, all Department wise services are listed. Choose the service you want to apply for and click the “Apply” link next to the service.
** On the Fresh Application screen, go to Alternative 2(Online Form application) section and click on “Click here to fill the application form online” link.
** Fill the form and click “Submit”.
** Click “View Saved Application” link, select the application and click “Upload Supporting Documents” button.
** Upload the supporting documents.

Offline form Instructions :
** On the State Portal of Punjab, click on Citizen Login link.
** If you are a new user, click on “New User” link, else login with your** ** Username and Password.
** After logging in for the first time, user will be asked to update their profile.** Please update it.
** Click on “Fresh Application” link in the left menu.
** On the next screen, all Department wise services are listed. Choose the service you want to apply for and click the “Apply” link next to the service.
** On the Fresh Application screen, go to Alternative 1(Offline Form application) section and click on Form Name to download the e-Form.
** Fill the e-Form in offline mode (Internet connection is not required to fill e-Form).
** Login to the application and on the Fresh Application screen, go to Alternative 1(Offline Form application) section and click on “Click here to upload the filled form” link.
** Browse to the filled e-Form and click on “Add” button.

Note :
1. The “Personal Details” section is non-editable. To modify Personal Details, please update your profile in the application and then download the Form again
2. All fields highlighted in red are required fields
3. Please refer the last page for a list of required documents
4. To type in Punjabi, please install Punjabi keyboard by going to Control Panel >> Region and Languages >> Keyboards and
Languages Click “Change Keyboards” and select “Punjabi”. Select Punjabi from the taskba
