Process :
The applicant format 2, the following documents related to the licensing officers will be present at the scheduled date and time

** Form No. 1 ( download ) in the therapeutic Swgoshna (physical fitness Manifesto)
** Transport (Professional) category of vehicles Learning licenses Form No. 1-A ( download ) the medical certificate shall be issued by the government serving doctor.

Note :
Non-transport category license for motor vehicles is given in figure 1. The only medical Swgoshna sheet form.

** 2 copies of recent passport size photographs of the applicant.

Residence certificate attested copy of any of the following evidence :
(I) photo ID
(ii) a copy of the electoral roll
(iii) life insurance policy,
(iv) Passport
(v) the Central or State Government or a local body of evidence of a pay slip issued by the office.
(Vi) the base card

The following age certificate from a per-
(I) School certificate
(ii) birth certificate

(v) base card

** Learning license for transport of vehicles at least the 8th pass attested copy of the certificate is required.

Note :
