Permanent Registration :

The Applicant is required to submit the following documents in original with its photocopies:
1. Original Provisional Registration Certificate.

3. All The Mark Sheets of M.B.B.S. Examination.
4. Internship Completion Certificate from the Dean / Principal of the Medical College.
5. M.B.B.S. Degree / Provisional Degree.
6. Three recent passport size Color photograph (not more than of 5cm x 4 –1/2 cm)
7. For registration fees & penalty amount, please see the prescribed fees chart.
8. The amount of fee, for obtaining any N.O.C. /Confirmation as fixed by the concerned university / institution, shall be payable by the applicant separately. as detailed in the fee statement.

Reciprocal Registration:
The Applicant is required to submit the following documents in original with its 2 photocopies:
1. Original Permanent Registration Certificate of the parent State Medical Council.
2. High School / Higher Secondary mark sheet in support of Date of birth,
3. Final M.B.B.S. mark sheet of Part I, II,
4. Internship Completion Certificate from the Dean / Principal of the Medical College.
5. M.B.B.S. Degree / Provisional Degree.
6. 3 recent passport size Color photograph (not more than of 5cm x 4 –1/2 cm)
7. For registration fees & penalty amount, please see the prescribed fees chart.
8. The amount of fee, for obtaining any N.O.C. /Confirmation as fixed by the concerned university / institution, shall be payable by the applicant separately. as detailed in the fee statement.
9. The Registered Medical Practitioner of Chhattisgarh Medical Council is required to submit the letter of N.O.C. from the Registrar, Chhattisgarh Medical Council, Raipur.

Provisional Registration:
The Applicant is required to submit the following documents in original with its photocopies :
1. High School / H.S.S. mark sheet / Certificate in support of Date of birth
2. Final M.B.B.S. mark sheet of Part I & II.
3. Internship under doing certificate in original from the Dean of concerned Medical College
4. 2 Recent passport size Color photograph (not more than of 5cm x 4 –1/2 cm)
5. For registration fees & penalty amount, please see the prescribed fees chart.
6. The Registered Medical Practitioner of Chhattisgarh Medical Council is required to attach a letter of N.O.C. of Chhattisgarh Medical Council.
7. The amount of fee, for obtaining any N.O.C. /Confirmation as fixed by the concerned university / institution, shall be payable by the applicant separately. as detailed in the fee statement.

Duplicate Registration :
The Applicant is required to submit the following documents in original with its 2 photocopies:
1. Photocopy of Registration Certificate
2. High School / Higher Secondary mark sheet in support of Date of birth,
3. Final M.B.B.S. mark sheet of Part I& II,
4. Internship Completion Certificate from the Dean / Principal of the Medical College.
5. M.B.B.S. Degree / Provisional Degree.
6. F.I.R. (for lost / Theft / missing / accident)
7. An Affidavit in the enclosed Proforma –P
8. 2 recent passport size Color photograph (not more than of 5cm x 4 –1/2 cm)
9. The prescribed fee (see fee chart).

Additional Registration of P.G. Medical Qualification:
The Applicant is required to submit the following documents in original with its 2 photocopies:
1. Original Permanent Registration Certificate of MBBS of the Chhattisgarh Medical Council.
2. Original recognized Post Graduate Degree / Diploma.
3. For registration fees & penalty amount, please see the prescribed fees chart.
4. The amount of fee, for obtaining any N.O.C. /Confirmation as fixed by the concerned university / institution, shall be payable by the applicant separately. as detailed in the fee statement.
5. Name of your Medical College with a proof of authentic document.

Requirements For Change In Surname (Only Female Case):
1. An Application on plain paper addressing to the Registrar, Chhattisgarh Medical Council.
2. An Affidavit or Marriage Certificate
3. Original Registration Certificate (Without lamination)
4. The prescribed fee (see fee chart).

Requirements For Change In Address:
1. An Application on plain paper addressing to the Registrar, Chhattisgarh Medical Council.
